We can help people leaving hospital who have no support network and may be experiencing homelessness or other complex challenges.

We help by:

• Ensuring patients get their medication and attend follow-up appointments Supporting access to housing,
• Universal Credit and other essential services
• Providing emergency items needed before or after discharge
• Offering outreach and practical help for those without family or support networks
